Intake Form

We ask you to fill out our intake form. We do this so that we can have some background information ahead of time, and identify your particular areas of concern. The more information you provide, the better prepared we will be. If you choose not to share something on the form, please indicate that, and we will respect your privacy.

The Intake Form is an interactive PDF file. Please follow these steps to access, fill out, and submit the intake form:

1. Download our intake form. Relationship Specialists' Intake Form

2. It is essential that you rename the form (for example "Jane Doe's intake form").

3. You may now fill out the form on your computer.

4. Save the completed file with its new name to your desktop.

5. Before submitting the form, be sure you have saved the copy you filled out.

6. To submit the form, enclose it in or attach it to an email addressed to Bob.Handelman@Valley.net.
